Process & Eligibility
A non-refundable $200 community enrollment fee is due upon acceptance into our program.
Cozy Pathways 4-Step Process:
- Step 1: Fill out the quick INTAKE form online.
- Step 2: Receive a call from our team within 24 hours to schedule a consultation.
- Step 3: Housing fit assessment.
- Step 4: Move-in coordination.
Eligibility Requirements:
- Identification: Valid government-issued photo ID
- Proof of Income: Documentation showing steady, reliable income
- Self-Sufficiency: Ability to live independently safely
- Public Safety Screening: Limited criminal history search

Accepted Payment Guidelines
Accepted Payment Formats: SSI, SSDI, Private Pay, Cash, and Agency Sponsor Payments welcome. Note: Housing Vouchers are not accepted.
